Benefits

ARA-290 (cibinetide) is a synthetic 11-amino-acid peptide derived from the tissue-protective 'helix-B' region of erythropoietin (EPO) — engineered to keep EPO's tissue-protective, anti-inflammatory, and nerve-protective actions while removing its red-blood-cell-stimulating effect entirely. It activates the innate repair receptor (IRR), a receptor complex that's selectively upregulated in injured or inflamed tissue and largely absent in healthy cells, triggering anti-apoptotic (anti-cell-death) signaling and suppressing local inflammatory cytokine cascades without the cardiovascular risk associated with standard EPO.

Uses

ARA-290 has been researched almost entirely around neuropathic pain and nerve repair, with its clearest human evidence in sarcoidosis-associated small fiber neuropathy and diabetic neuropathy, plus preclinical exploration in diabetic retinopathy, traumatic brain injury, burns, and shock-induced multi-organ failure. It retains FDA Orphan Drug and Fast Track designations for sarcoidosis-related neuropathic pain, though it is not approved for any use.

Published Research

ARA-290 carries genuine Phase 2 human trial data — a real distinction from most research peptides, which typically have no controlled human trials at all. A 2012 randomized, double-blind pilot study in sarcoidosis patients (published in Molecular Medicine) and follow-up Phase 2 trials found that 4 mg daily subcutaneous injections over 28 days increased corneal nerve fiber density (by 697 μm²/mm² over placebo at day 28) and significantly reduced neuropathic pain scores. A separate 2015 Phase 2 trial in type 2 diabetes patients (Brines et al., Molecular Medicine) reported statistically significant improvements in HbA1c (p=0.002), neuropathic symptom scores (p=0.03), and cholesterol/triglyceride markers over 28 days. Animal research (Swartjes et al., 2014, Molecular Pain) found ARA-290 produced pain relief lasting up to 20 weeks after a single injury model, with a clear dose-response relationship.

Despite these genuinely positive early results, no Phase 3 trials were ever completed — sponsor Araim Pharmaceuticals ceased operations, and development has stalled with no active trials as of 2026. This is worth being direct about: ARA-290's development didn't stop because the science failed, it stopped because the company behind it did, leaving real clinical signal stranded without the larger confirmatory trials needed for approval.

Why More Research Is Needed

ARA-290's gap is unusual in this series: it isn't that the compound lacks evidence, it's that genuinely positive Phase 2 human data for a specific, narrow indication (sarcoidosis-associated small fiber neuropathy) was never followed up with the Phase 3 confirmatory trials that would be required for approval or broader clinical use. Its human trial base, while real, remains limited to two relatively small patient populations (sarcoidosis and type 2 diabetes), so its relevance to nerve pain from other causes is unconfirmed. With no active sponsor or ongoing trials as of 2026, further human research would require new institutional or pharmaceutical backing to pick the program back up — a funding and sponsorship gap rather than a scientific one.
