Benefits
NAD+ (nicotinamide adenine dinucleotide) is an essential coenzyme present in every human cell, central to energy production (ATP generation via oxidative phosphorylation), DNA repair, calcium signaling, and gene expression. Levels decline meaningfully with age — human skin studies show roughly a 70% decline and liver studies show about a 30% loss over the lifespan. Research links restoring NAD+ activity to metabolic, mitochondrial, and cellular repair pathways, with early clinical signals in fatigue reduction and quality of life.
Uses
NAD+ and its precursors (nicotinamide riboside/NR, nicotinamide mononucleotide/NMN) are researched for healthy aging and longevity support, metabolic health, mitochondrial function, chronic fatigue syndrome, and — more recently — long-COVID fatigue and cognitive symptoms. It's widely offered through IV therapy clinics for general wellness and anti-aging purposes, alongside oral supplement forms.
Published Research
NAD+'s evidence base is broad but notably indirect, and the distinction between what's actually been tested matters enormously here. A 2026 PRISMA-guided systematic review of 113 studies (33 human, 80 rodent) found oral precursors — NR and NMN — consistently raised NAD+-related biomarkers and were generally well tolerated, but functional, metabolic, and vascular outcomes were heterogeneous and often null. Critically, that same review found no eligible outcome trials evaluating IV or intramuscular NAD+ itself for anti-aging or wellness use — the specific form most commonly marketed and administered in wellness clinics. The one intravenous NAD+ pharmacokinetic study identified contributed only safety data, not efficacy evidence.
A 2025 double-blind trial testing oral NAD+ on cognition found it did not outperform placebo, though the authors noted medium effect sizes suggesting larger trials might detect a true effect if one exists. More positive signals exist elsewhere: a 24-week randomized trial in 58 long-COVID patients found nicotinamide riboside supplementation showed potential improvements in fatigue, sleep, and mood, though the study's primary group comparisons didn't reach statistical significance and results were described by the authors as exploratory. A separate systematic review of NAD+/NADH supplementation broadly concluded it appears safe with mild side effects, and some included studies reported improved fatigue and quality of life in chronic fatigue syndrome patients.
Why More Research Is Needed
NAD+'s gap is worth being unusually direct about, since it's a specific and important distinction: a significant portion of the human trial evidence backing 'NAD+' claims is actually about oral precursor compounds (NR, NMN), not NAD+ itself, and a comprehensive 2026 review found literally no outcome trials testing IV/IM NAD+ — the form most commonly sold and administered in wellness settings — for the anti-aging and wellness benefits it's associated with. Where direct trials of NAD+ or its precursors do exist, results on functional outcomes are frequently null or inconsistent even when the underlying NAD+ biomarkers improve, meaning raising NAD+ levels doesn't reliably translate to the outcomes people are actually seeking it for. Larger, longer, properly controlled trials with clinically meaningful endpoints — specifically for the IV/IM form as it's actually used in clinics — are the explicit next step researchers themselves are calling for.
